What to bring to your appointment:

• Your Vermont Medical Marijuana Patient's Card, indicating PhytoCare Vermont as your designated dispensary.

• A locking container in which to transport any cannabis products.

• NEW PATIENTS: Please be sure to bring one additional state or government issued form of Identification.

Cancellation Policy:

If you need to cancel your scheduled appointment, we ask that you do so at least 24 hours in advance.
